As the Community Health Worker, you will:
•Coordinate patient care with patient, family, and ancillary/community services to promote quality and cost-efficient care for assigned caseload
•Assist patients with enrollment and/or referring them to various health and social service programs
•Assist patients to schedule medical appointments to include connection with transportation resources
•Provide follow-up to patients, by phone, home visits and visits to other settings where patients can be found
•Conduct home visit to ensure that the patient has what is required to recover safely at home and to conduct safety checks (e.g., check for fall risks) and prepare for follow-up appointments with provider
•Bachelor of Science in health education, health promotion, or related field
•1+ years’ experience working in a community-based setting with knowledge of local resources and system navigation
•Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance
•Active American Heart Association Basic Life Support (BLS) certification required
•Empathic toward individual’s complex health needs and their families
•Strong communication and customer service skills
•Knowledge of community resources
•Strong writing skills
•Strong organizational skills
•Ability to work in a fast-paced environment while managing multiple priorities and maintaining professionalism
•Excellent time management skills
•Ability to problem solve and exercise good judgment
