The Diabetes Peer has a deep understanding of their community and is a cultural mediator between their church/faith community and the Baylor Scott & White Health system. They work closely with Community Health and Diabetes Educators to provide basic diabetes and health education, informal counseling and self-management support that is culturally appropriate to their specific setting.
ESSENTIAL FUNCTIONS OF THE ROLE
Assists with the tailoring of a faith-based diabetes educational program that is relevant to the unique beliefs of the faith community of the Diabetes Peer.
Promotes the provision of culturally competent care by educating the Diabetes Health & Wellness Institute (DHWI) staff about beliefs and practices within their faith community.
Recruits fellow members of their faith community who have diabetes to participate in a diabetes education program.
Conducst a comprehensive diabetes education program and evaluate the achievement of learning objectives of each participant.
Collaborates with Community Health and Diabetes Educators in supporting prescribed therapies.
Links people with appropriate community resources and services available from DHWI.
Attends 160 CHW training course provided by DHWI.
KEY SUCCESS FACTORS
General understanding of diabetes care modalities, therapies, terminology and equipment.
Must be able to communicate thoughts clearly; both verbally and in writing.
Ability to effectively use a variety of teaching methods.
Knowledge of the geographic area, the community and its resources
Cultural competency related to health practices
Committed, dependable, adaptable, creative, resourceful and has strong desire to help the community.
Able to interact positively with patients as needed.
General computer skills, including but not limited to: using required software applications, data entry, information security, electronic medical documentation, hand held scanning and email.
