What are we looking for in a Nurse Care Manager?
The Nurse Care Manager coordinates and supports the provision of medication assisted treatment services for incarcerated clients within the Kittitas County Jail. This program is a collaborative effort to support individuals who are incarcerated and experiencing opioid dependency by providing treatment while in jail and coordinating their transition to outpatient treatment services upon release to avoid gaps in care and maintain positive momentum once in the community. Working in coordination with other mental health and substance use treatment providers at the jail, the Nurse Care Manager is responsible for collaborative treatment and discharge planning, intake and induction of new patients, administering medications based on prescriber orders, and providing patient education. This position is responsible for managing the initiation, maintenance, and post-release transition process for medication assisted treatment within the jail setting, including ensuring that each treatment provider is involved appropriately throughout the process. The Nurse Care Manager position requires an understanding of substance use disorders and their treatment, supports the evidence-based model of medication-assisted treatment, and understands the potential risks and thus need to follow exactly all protocols & testing to provide safe treatment. Excellent communication and care coordination skills are essential in this role.
