The Woodlands Hospital based freestanding emergency centers throughout the Greater Houston area provide a more streamlined process for hospital admissions, if needed, than non-hospital affiliated clinics and urgent care centers. If needed, we refer and transport patients to the nearest CHI St. Luke’s Health hospital for direct admittance, providing the resources of a larger facility at a convenient location.

A Registered Nurse responsible for coordinating services across the continuum of care within the Cardiovascular Service Line, by monitoring, evaluating and modifying the treatment plan to achieve optimal patient outcomes. The RN employs critical thinking to integrate objective data with knowledge gained from initial and ongoing assessments including the objective and subjective experiences and health care history of each patient. The RN will also use this critical thinking process to apply the best, evidenced based practice and research data to interdisciplinary care coordination from admission to discharge and transition planning. The RN will review clinical documentation to assess the accuracy, specificity and completeness of physician and clinical caregiver documentation and to identify if clinical findings suggest the presence of other conditions that are not explicitly documented in the patient’s health record. The RN will continually evaluate quality, risk and patient safety to optimize outcomes. The RN will integrate other requirements as outlined by CMS and The Joint Commission to assure that patient care is provided and documented to reflect minimum requirements and standards.
Plan, coordinate, and implement the care of cardiovascular patients
Review the clinical progress of patients, and coordinates communication to the referring physician and interdisciplinary team
Facilitates the development of patient specific teaching plan
Review the patient’s current and past health information, relay information and make referral as requested by the physician and interdisciplinary team
Assist staff with the coordination of the patient clinical record and associated studies
Demonstrate knowledge of protocols and appropriate patient selection
Will work with the quality team as needed and appropriate, will be assigned other related tasks as directed
Review medications appropriate to the patient
Acts as educator for the patient(s) and family members on appropriate medication use and explaining the physician’s orders
Serves as a resource for the hospital staff regarding the care of cardiovascular patients
Educate the patient and family members regarding planned procedures, medications and treatments
Make referrals as appropriate as noted by the physician
Coordinate information as applicable
Structural Heart team members
Electrophysiology team members
Answer patient and family member inquiries and field questions timely and appropriately
Participant in creating an environment that implements the Mission, Vision, and Values of the organization
Encourage collegiality when integrating CV services with other departments for abnormal findings.
Actively participate in quality improvement activities and encourages service excellence through recognition of its occurrence, and through recovery, by addressing deficiencies
At least 5 years full-time experience in a cardiology or critical care unit in an acute care setting. A combination of education and experience, which provides the above knowledge, skills and abilities will be considered
Knowledge of cardiac catheterization lab procedures, STEMI, TAVR, Watchman, Electrophysiology, AICD/ICD
Knowledge of national registries such as NCDR, TVT, STS a must
Current unencumbered Texas RN licensure. BSN is preferred
To perform this job successfully, an individual must be able to perform key activities satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. The applicable laws, rules, regulations, policies and procedures pertaining to applicable Joint Commission and HIPPA regulations. Delivering complex, detailed, verbal information to patients/families, managers, physicians, staff and others. Intermediate knowledge of computer word processing, spreadsheets, and other database programs including Excel and Google Sheets; ability to write reports and do graphical analysis. Correct English usage, including spelling, grammar and punctuation. Self starter, detail oriented and ability to interact with physicians. Ability to coordinate multiple tasks, assignments and responsibilities. Ability to maintain confidentiality of reports and legal documents.
