At Vidant Home Health and Hospice, we believe that no family should have to face terminal illness or the death of a loved one alone. Our hospice team provides support for patients and the families of patients with a limited life expectancy. Using a team approach, we work to meet the special physical, emotional, social and spiritual needs of our patients and families.
Position Summary
Provides direct skilled nursing services as described in the patient’s plan of care and manages the interdisciplinary care of a caseload of patient to achieve optimal quality and financial outcomes.
Responsibilities
1. Demonstrates the knowledge and skills necessary to provide service appropriate to the age of the patient.
Provides individualized nursing care as indicated to home health and hospice patients.
Performs on-call duties as assigned.
2. Assesses patients and identifies needs that encompass the total well being of the patient and family unit.
Uses independent professional judgement to assess, identify needs, and develop a holistic plan of care encompassing medical/physiological, diet, activity, rest/leisure, home safety, infection control, patient’s rights, effective perception of reality, sensory integrity, freedom from pain, psychological and/or spiritual well-being, ambulation, adaptation to stress, responsibility in health maintenance, self-care and patient/family education and appropriate discharge and/or end of life planning.
Determines the nature and severity of problems identified, the prognosis for improvement, and the recommendations for care and services needs.
3. Develops and revises an effective plan of care in collaboration with the patient, family, referring physician, and multidisciplinary team.
Manages the development of a plan of care based on assessed needs in collaboration with the patient, family, physician, and other team members.
Evaluates the plan of care to determine whether the goals are achieved within the most cost effective and efficient manner.
Manages the care of an assignment of patients and provides services based upon the plan of care.
Evaluates, documents progress, and revises patient’s plan of care as needed, and at minimum of every 60 days in home health, through collaboration with the physician and/or interdisciplinary team.
Continually evaluates the patient in regard to appropriate criteria and collaborates with the attending physician in certifying and recertifying the patient’s plan of care for service in accordance with service Medicare/Medicaid benefit requirements.
4. Instructs the patient/family about care, treatment, and services and the individualized plan of care as ordered by the physician.
Counsels the patient and family regarding services, end of life issues (hospice), patient’s rights and responsibilities, reimbursement constraints and any financial responsibilities of the patient/responsible party.
Teaches patients, family members, caregivers, nursing assistants, and other health team personnel in performing appropriate therapeutic and/or palliative treatments/interventions.
Instructs family members, caregivers how to care for the patient to facilitate independence and optimize quality of life in the patient/family unit.
5. Observes, records and reports to the physician any reaction to treatment or changes in the client’s condition.
Documents a comprehensive evaluation and informs physicians, patient/family and team members of results and recommendations.
Completes chart documentation in a timely manner according to policy.
Coordinates flow of paperwork involved in patient care.
6. Performs supervisory visits/oversite of Nursing Assistants and LPNs per regulatory requirements.
7. Assists in the education of medical students/residents, nurses, nursing assistants, and others as identified.
Serves as a preceptor in the clinical training of medical students/residents, nursing students, nursing assistants, and others.
Assists with the orientation of new staff and others as assigned.
8. Demonstrates professionalism and service excellence.
Adheres to Agency’s policies and procedures.
Maintains current knowledge and skill base necessary to the provision of quality home health and/or hospice nursing.
Participates actively in patient care conferences, interdisciplinary team meetings, regularly scheduled staff meetings and in-service classes.
Uses tact and courtesy in all interactions with staff, patients, families and referral sources.
Promotes a positive image of the Agency and supports Administration and Vidant Health in its goals and objectives.
Handles inquiries and complaints discreetly and effectively per policies and procedures.
Demonstrates awareness of hospice and/or home health, HealthAccess, and Vidant Health policies, including confidentiality and HIPAA.
9. Patient Safety and Quality Improvement
Follows Patient Safety guidelines, organizational policies and procedures, professional standards of care, best practices, and regulatory requirements related to patient care quality.
Assists in the development of indicators and conducts monitoring of CQI activities and evaluation of service provision.
Contributes to achieving the agency’s quality outcomes.
Participates actively in Patient Safety and Quality Improvement activities.
Consistently “Calls the Question” regarding patient safety or quality issues, and maintains a safe environment for patients, visitors, and employees.
Consistently uses good judgment and thinks critically in order to make correct decisions.
Minimum Requirements
BSN preferred.
Licensed as a Registered Nurse by the N.C. Board of Nursing.
More than 2 years progressive clinical experience preferred.
Evidence of skills in:
Effective communication
Organization
Infusion therapy and other technical skill validation
Sound assessment and professional judgement
Demonstrates flexibility.
For Home Health and Home Hospice RN positions:
Must maintain a valid North Carolina driver’s license or be licensed in the state you reside and maintain the Vidant Health required minimum insurance limits (Clinical home staff).
Must maintain adequate/dependable transportation in order to perform specific job functions.
Basic Life Support (BLS) Certification
